Waiver I, the parents/guardians, give my child permission to participate in the youth Basketball program offered by the Meridian Parks and Recreation. I hereby agree to hold the City of Meridian and its employees harmless from any liability that may arise from incidents, accidents or COVID-19 related sickness involving my child while participating. I understand there are risks present and I take full responsibility of those. I understand if my child decides not to play after he is placed on a team, I will not receive a refund of my registration.

$20 Uniform Fee
Paid to All-Star Team Sports at the Skills Evaluation
To receive a uniform, the player must attend one of the Player Skills Evaluations at TBD.
